How to Use These Forms

1Hold the SMC meeting and fill in the SMC Resolution with the approved amount for each component. Get the members' signatures.

2Issue the Sanction Order and Work Order with the RC number, date, vendor name and Vendor ID.

3After the supply or work is done, collect the Vendor Bill Submission Letter along with the original bills.

4Prepare the Non-Drawal Certificate and the Payment Voucher, then sign and seal them.

5Keep all forms, bills, photographs and utilisation certificates together for record and audit.
