This document serves as a cornerstone of integrity for any record attempt, ensuring that all guidelines are scrupulously followed and impartially observed.

The Witness's Solemn Declaration

At the heart of the witness statement is a declaration of impartiality. A witness must affirm that they are "not associated with, or related to, the record organizers or participants" and have "nothing to gain from the final outcome of the attempt". This fundamental principle upholds the credibility of the record-breaking endeavor. By signing this declaration, the witness confirms their role as an unbiased observer of the Guinness World Records™ attempt.

Key Information Required from a Witness

The template systematically gathers essential information from the witness to ensure transparency and accountability:

• Record Title: The specific record being attempted must be clearly stated.

• Contact Details: This includes the witness's first name , last name , organization (if applicable) , nationality , email , and telephone number. An "Application Reference Number" is also requested.

  Witness's Observations: The statement requires a detailed account of what the witness "see / measure / evaluate" , with an emphasis on how "all the record guidelines" were observed and followed.

• Field of Expertise: To lend weight to their observations, the witness must specify their "field of expertise" , which serves as the "Reason for witnessing the record attempt".

• Final Measurement: Any critical final measurements relevant to the record must be recorded.

• Location and Time of Attempt: The venue , city/town , and country where the record attempt took place are essential details. Furthermore, the exact dates and times the witness was present must be included.

Completing the Statement

The witness statement concludes with an acknowledgment that the document was completed by the individual and a willingness to be contacted by Guinness World Records for further discussion. A handwritten signature and the date (dd/mm/yyyy) are mandatory to validate the statement.
