For retired government employees in Andhra Pradesh, securing continued medical coverage is a top priority. The Employee Health Scheme (EHS) provides a seamless transition from your service health card to a dedicated Pensioner Health Card, ensuring access to cashless treatment under the Dr. YSR Aarogyasri Health Care Trust.

After retirement, your employee EHS card remains valid for only six months. It is crucial to apply for your new pensioner card within this window to avoid any gap in medical benefits.

This comprehensive guide will walk you through the entire online application process, from initial setup to downloading your final digital health card.

Phase 1: Essential First Steps Before You Apply Online

Before you can even log in to the EHS portal, you must initiate the process by deactivating your old employee health card.

  1. Compose an Email: Send an email to ehs@ntrvs.ap.gov.in.

  2. State Your Purpose: Clearly state that you are a retired employee and wish to deactivate your employee health card to apply for a pensioner health card.

  3. Attach the Following Documents:

    • A scanned copy of your PPO (Pension Payment Order).

    • A scanned copy of your Aadhaar Card.

    • Your latest Pension Pay Slip.

    • A Self-Declaration Letter stating your request.

Once the administration processes your request, you will be able to proceed with the online application.


Phase 2: Account Setup on the EHS Portal

Your next step is to set up a password for your account.

  1. Navigate to the official EHS portal: http://www.ehs.ap.gov.in.

  2. Click on the "Forgot Password" link.

  3. Fill in the required details:

    • User ID: This is critical.

      • For pensioners who retired before 2020: Use your PPO ID / Treasury ID.

      • For pensioners who retired after 2020: Use your CFMS ID.

    • Login Type: Select "Pensioner" from the dropdown menu.

    • Captcha: Enter the security code shown in the image.

  4. You will receive an OTP on your registered mobile number. Enter the OTP to verify.

  5. After successful verification, you will be prompted to create a new, secure password.

You can now log in to the portal using your User ID and this new password.


Phase 3: The Online Application - A Step-by-Step Walkthrough

Before you start filling the form, ensure you have the following scanned documents ready. Each file should not exceed 200 KB in size.

  • For Pensioner: PPO, Passport-size Photo, Aadhaar Card.

  • For Dependents (Spouse, Children, Parents): Passport-size Photo, Aadhaar Card.

Step 1: Initiate the Application After logging in, look for the "Registrations" tab on the left-hand menu. Click on it, then select "Initiate Health Card / View Application." You will be asked to enter your Aadhaar number and verify it with an OTP to retrieve your details.

Step 2: Fill the Application Form The "Application for Pensioner Card" will open. Fill out all the sections accurately. Fields marked with an asterisk (*) are mandatory.

  • Personal Details: Retirement type, name, date of birth, gender, marital status, date of retirement, etc.

  • Address Details: Your complete residential address.

  • Identification Marks: Provide at least one distinct identification mark.

  • Last Posting & Pension Office Details: Use the dropdown menus to select your last place of work and your designated pension office.

  • Save Your Progress: Click the "Save" button frequently to avoid losing your data.

Step 3: Add Your Dependents After saving your personal details, the "Family Members Detail" option will appear.

  • Click the "Add Beneficiary" button.

  • Enter your spouse's Date of Birth and click "Submit."

  • A new window will open. Verify your spouse's details using their Aadhaar OTP.

  • Complete their profile and upload their scanned Aadhaar card and passport-size photo. Click "Save."

  • Repeat this process for all eligible dependents.

Step 4: The Two-Part Submission Process This is the most important part of the application. Follow these steps carefully.

  1. Initial Upload & Submission: In the main application, click on the "Add Attachments" button. Upload your Aadhaar card, PP Photo, and PPO scan. DO NOT upload the "Signed Application Form" yet. Click "Save," and then click the "Submit Application" button.

  2. Print and Sign: After submitting, the "Print Application" button will become active. Click it to download your completed application form, which will include your photo. Take a printout, fill in the Station and Date, and sign it in the designated space.

  3. Final Upload & Submission: Scan the signed application form. Log back into the EHS portal, go to "Add Attachments," and now upload this "Signed Application Form."

  4. Finally, click the "Submit Application" button one last time.

Your application will now be forwarded to your respective STO/DDO for approval. You will receive a confirmation message.

Scanning and Saving Documents

  1. PPO received from AG office

  2. Passport size photograph of the pensioner

  3. Aadhaar card of pensioner and all dependents

  4. Passport size photographs of all dependents

  5. Dependents’ Aadhaar cards

Phase 4: Downloading Your Digital Health Card

Even if STO approval is pending, the system often generates the Smart Health Card. You can log in to the portal after a few days to check the status and download the digital card. It's recommended to print it, laminate it, and keep it safe for all future medical needs.
